Pin Reset Blocked or Locked:

If your CAC card is currently blocked or has been locked, then come by the CAC DEERS office this transaction doesn't require an appointment.

Appointment Times:

Each appointment time that is scheduled will accommodate only one customer, therefore if you need multiple ID cards produced each person will need an appointment time.

What to bring:

When requiring a new ID card issued - anyone 18yrs old or older must have 2 forms of a valid Government ID (Expired Military ID will be accepted as a form).

Adding/Removing spouse:

Adding spouse to DEERS you will need to provide Marriage Certificate (Original or Certified copy), Social Security Number card (Original). Also, we will need to see at least 2 forms of valid ID (Driver's License, Passport or Passport card, SSN Card). Removing spouse from DEERS will require a Divorce Decree (Original or Certified copy).

Adding Child:

Adding child to DEERS requires Birth Certificate or Hospital proof of Birth (Original or Certified copy), Social Security Number Card (Original form)

100 Percent Disabled American Veteran:

Disabled American Veteran (DAV) will need to provide DD form 214, Veterans Administration letter that states 100 percent disability and are eligible for Base Exchange and Commissary privileges. To be issued DAV ID card you will also need to provide two forms of valid Government ID.
