To determine eligibility and identify the right supports, our team completes a MNCHOICES assessment. This comprehensive assessment helps us understand each person’s needs and connect them with services that make it possible to remain at home safely and comfortably.
A MnChoices assessment is a comprehensive, face-to-face evaluation used to understand an individual’s health, daily living needs, and overall safety at home. It is required to determine eligibility for programs like the Elderly Waiver (EW) and Alternative Care (AC).
During the assessment, a trained assessor meets with the person (and their caregiver, if they choose) to talk through areas such as mobility, personal care needs, memory, medication management, home safety, and support systems. The assessment helps identify which services or supports the person could be eligible for to help the individual remain living independently in their home or community.
MnChoices is offered at no cost, and it provides a clear roadmap for accessing long-term care options that fit each person’s unique situation.

Care Coordination
Older adults enrolled in a county‑based health plan can receive support through care coordination. Our coordinators help ensure older adults have the services and resources they need to remain healthy, safe, and supported in their homes or community.
Our care coordinators help arrange assessments, identify needs, and work with the individual to create a person‑centered care plan. They help arrange and coordinate medical appointments, long‑term services and supports, and other essential resources such as housing or food assistance. With permission, they communicate with providers, agencies, and members of the person’s support network to keep everyone informed and aligned.
Care coordinators also maintain regular contact, monitor changes in health or functioning, remind individuals about preventive care, and collaborate with waiver case managers when Home and Community‑Based Services are involved. As a local and accessible resource, our care coordinators offer ongoing guidance to help older adults stay connected to needed services and maintain their health and independence.
